Overview
At this session we will share the example of a trusted assessor service making a big difference to residents and acute hospitals in Dorset. It has led to 80 people every month being supported back to the place they call home, and to date there have been no failed discharges for people supported by the trusted assessors. At this session, you’ll learn more about the key elements needed to develop a similar service.

- practical steps to develop similar schemes and to measure its success and value.
Key speakers
The session will be delivered by Anna Knight, Dorset Care Association and Sarah Sewell, Dorset Council Commissioner, who working in partnership, have embedded a successful trusted assessor service, that now supports all acute and community hospitals across Dorset.
Who should attend?
Health and local authority commissioners, particularly those responsible for hospital discharge pathways, and / or responsible for strategic market partnerships.
